To better assist our office in preparing your Divorce papers, there are
several forms that need to be completed. The forms are listed on the left
side of the page and are in Microsoft Word format. You can download the
forms and complete them electronically or print them and hand write your
answers. A brief description of the forms:
Questionaire
Brief information regarding your contact information, your spouse and where they
may be served/contacted. Information about your marriage and separation.
The information regarding your employment will be used primarily to determine
child support and if applicable, alimony. The remaining information is to
determine what the primary issues will be for your divorce. Please let
your attorney know if there are issues that need to be addressed that are not
listed.
Vital Statistics
This is a required governmental form. It does not go in the court file,
but is used by the Court to complete their reporting requirements to the
government.
Financial Affidavit
Nebraska requires this Affidavit to be completed prior to requesting any
temporary child support or temporary alimony.
Child Affidavit
Effective January 1, 2008 Nebraska is requiring this affidavit to be completed
if any children under the age of 19 are effected by the divorce proceeding.
Domestic Violence
Effective January 1, 2008 Nebraska is requiring attorneys to screen all divorce
clients for domestic abuse. Your attorney will want to discuss the items
in this document. Do not be offended. We must ask every client about
domestic abuse. It is in your best interest to accurately and truthfully
answer the questions. The attorneys are not required to disclose this
information to anybody.
